Hipaa: Photo Uploads are not showing.

Hello there,

We are not able to access photo uploads in our forms except on rare occasion. Clients are confirming they have uploaded their photo. Please advise on how to fix this recurring issue.

I've tried it on our test hipaa account, but I'm not able to replicate the issue.

When viewing the uploaded files, may I know what error do you get? Are you viewing it from Jotform submission page or a link that is included in the notification? 

If you are still able to replicate it, kindly share to us one file link.

When I pull up submissions in jot forms for the 14 and over intake form the insurance card is supposed to be uploaded as a photo as part of this form. I do not see the upload anywhere. Because this is a HIPAA complain account I don't think I can share the client's intake form. This has been a recurring issue. Is there a way for you to complete 5 forms of our 14 and over form and see if that helps to duplicate the issue?

Unfortunately we cannot check your form's submission because of HIPAA compliance. I did test your form "Initial Paperwork for 14 Years and Older" and sent two test submission with file upload. I would suggest you to please check your form's submission and see if the uploads were received successfully.

Another issue I found in your form is that none of your form fields are required field. User can submit form without filling up your form. I would suggest you to please make few of your form fields a required field including file upload field and see if that solves your problem.
