Can I create forms that I use in my psychotherapy practice?

    I am interested in creating forms not only for my clients to fill out via my web page, but also ones that I can use, in house, to keep psychotherapy and treatment notes.  The latter would need to be either on a private website that only I have access to, or better yet, be able to be used on my desktop or laptop--but not be accessible to others?  

    Is this possible?

    Hi Jamie,

    Yes, for as long as you have an internet connection because our forms won't work without it.

    You can also password protect the form by adding the Page Break Tool under Survery Tools, adding the Password Box and using our Conditional Logic Tool. Example Form: http://form.jotformpro.com/form/23474749914970 (Codes to Proceed: VAL1)
