How do I create a template for the Form?

    Can I create a template that will be my "default" consultation form into which I will customize data for that particular patient-save and share it with the patient and still have the default form for the next patient? Since each patient will already have filled out their intake questionnaire, will there be a way to input their demographics from their questionnaire into my consult form?

    I am not sure whether I have understood your question correctly. Do you want to use the same Form for all the users or for each user you would like to create a separate Form?

    If you would like to populate the name or more details for different user and then send then you may use the Prepopulate feature where you may prepopulate data into the Form and generate a unique URL for each user.

    After the user has filled the details so do you like to edit the Form and enter more details just like office section?
