I want to design a form that has this:
Checkbox & Specialty Coffee Name 1
Two line description of coffee 1
Drop down box to specify grind (drip or expresso) or whole bean for coffee 1
Checkbox & Specialty Coffee Name 2
Two line description of coffee 2
Drop down box to specify grind (drip or expresso) or whole bean for coffee 2
Choose Payment: radio button for PayPal
radio button for Pay by Check
When I click your checkbox option, it gives me three check boxes but I can't add anything in between them. However, I saw a form on your support page that had checkboxes and then quantity boxes in between them.
Please help me learn how to create this type of form.

You must be referring to a form that is using a payment field.
Test this form http://www.jotformpro.com/form/30516524457958 . I added a payment field at the bottom.

Is there a way to put a description under the check box in a paymnet field?

There no option to add a description in our form builder but you can do that by getting your form source codes, find the checkbox items from the given codes, add in your description and after that, embed the edited codes.
