Error message on submit

Hello!

I've just made the following form for a work awards nomination form and I'm currently testing each option to make sure they work correctly. On the second form it won't let me submit because there are errors, and I have no idea why!

All fixed now thanks! I worked out that all of my questions had required input to answer so I took them out.
