This is the form I built:

It requires the * info, but only collects payment successfully when the person hits submit. The rest of the information on the form is not collected properly or saved on my account. We tested this with one person, and I received her payment, but no notification from jotform that I had a new response.
How do I fix this before our sales go live this Friday 5/31?

Can you please try to enable IPN in your PayPal account?
