Locked out of account

i am locked out of my account. I can’t get in to remove fields that need to be remove.   Please tell me what to do

Upon review of your account it was found form: http://www.jotform.us/form/90468569785175 was collecting ssn#s and driver's licenses. In order to reactivate your account we have to remove the fields for compliance.

Please be advised that this type of collection is not allowed unless you add the mandatory widgets in order the system to recognize a valid collection:

1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

Please add the widgets at your earliest convenience. Your account is active and your forms available.
