How to change a title

    Hello,

    The title of the form is Availability Request. However, TRAVEL INSURANCE GENERAL WAIVER is being displayed. How can I change it?

    I have controlled your form and there is no problem with your form title and page title.

    It is Availability Request.

    I kindly suggest you to clean your Whatsapp cache from your phone and then try to share it.
