We need to get access to the form created by a former employee

Good morning, I am working with a customer who uses a form created on JotForm that they'd like me to update. The form is here https://form.jotform.com/52935843004151. The issue is, this form was created by a former employee who no longer works and the company and therefore they do not have access to edit the form. Could something be done to help the customer?

The form is currently located in the account under the domain, mecu.org. The account has no activity as far as I could see. The domain also redirects me to another website, so I don't think the person is still working on the account. 

The best thing you can is to clone the form and use the cloned form for your clients.
