Anonymous forms for personal information

    Hi there,

    I would like to enable people to complete a personal history anonymously for data security purposes.  If the data was hacked, it could not be traced back to the person concerned.

    Is the only way to do this to give them a number to complete the form with.

    Then have them complete the form using the number and then I merge the two spreadsheets on my desktop computer for maximum security?

    As far as I understood, you will make two spreadsheets. First one will carry number-person matching and the second one will carry number-personal history matching. Later you will combine them.

    You can encrypt your data with a key file. Without this key file, no one can see your data.

    I think trusting JotForm is the best way for you. 

    On the other hand, will you match the numbers and people before the submissions? Then you will tell them their number, and they fill the form. Are you trying to do this workaround?
