I checked the form that you have shared above and could see that the Square integration is connected and the payment setting is in Live mode.

Please note that when the payment setting is in test mode, payment will not be charged during submission since you are just testing whether the integration is successful. When you switch to live mode, all payments that are done after switching to live mode only will be processed and not before that.
