Submissions done on a different URL

is it possible to have jotform to be set up so that the same jotform can be submitted with a different URL. 

Example: A salesrep wants to submit a form but we need to know which specific sales rep completed the form. 

How can this be done?

If I understood correctly, you would like for the URL to be different for each sales-representative that submits the form? If this the case, unfortunately, the only URL that can be different would be when you view each individual submission.
