How come we can not bring up our form

    We have an active form we used earlier this year and it says that we do not have any forms created? How come?

I can only assume that you are referring to another account and we can help you locate that account for you. Please provide us an of the following information so we can do so.

    1. Account username of the account.

    2. Any email address associated with the account.

    3. Form URL — any of your forms direct form link.
