Conditional Logic

Also, I want to add a condition where I only have to add information for the number of guests in a room.  So, if there are only 2 guests, I will only need to request info from two people.  If it is a family of 5, I will need info from all 5.  I am not sure how to add these conditions.

You can certainly make this work by using conditional logic on your form. You can add a field in where you ask How many guests will there be? If the limit of guest is only 5, make sure to add those information fields 5 times, once that is done you can get up conditional logic.

For example:

If How many guests? state Is Equal To value 5 do Show Multiple. Below there is a screencast of how you can achieve this.
