I have a number of requirements I was unable to accomplish:
1). Centering the Heading to the line.
2). Centering the whole form to the page.
3). Changing the font size in the heading without it affecting the text of the whole form.
4). Putting the logo before the text on the same line as the heading.
5). Even though I did use the option to make the font stand out in blue it did not save as such.
6). Centering the text in the text box option even though I did so it never saved as such.
7). The checkboxes are not in a vertical line on website as appear on Jotform.
8). Black dots appear all down form that do not appear on Jotform.
Your direction would be great.

To accomplished the number of requirement you listed, you will need to use a function called 'Inject Custom CSS'. This will allow you to change the design of your form based on your preferences.
Kindly refer to the link below to learn how you can use 'Inject Custom CSS'

I set up another form - "contact"
(http://www.all-in-israel.com/contact.html) and this one seems OK for
all intense and purposes except when the form is filled and sent the
confirmation mail I receive is in a foreign language and has a question
also not as the saved version depicts on the Jotform site. See SS

For the first issue, re-embed your form using iframe method: How to get your Form's Iframe Code, see if this will resolved everything.

Thanks for your reply.
Using the link you gave me I now have the form as I want to see it and
as long as I do NOT put in a "valid" Email address both the form and the
payment referral seem to work.
If you can examine and advise me or even correct for me the "bug" I
can start using the form on the site. I cannot send you a SS as every
time I try to capture the pop up text saying "plese enter a valid email
address" it immediately disappears.

I have injected some CSS codes to your form to achieved some of the styles you want on your form for your convenience. You can check this CSS codes when you follow this guide: How to Inject Custom CSS Codes to your Form
With regards to the pop-up message you've mentioned, are you seeing this on IE browser? I don't find any problems with your form so far. We would appreciate if you could take a screenshot, you can use printscreen (PrtSc) on your keyboard, paste it on mspaint and then upload the image here: https://cms.jotform.com/help/image_upload.php?naked=true, post the Image URL on this thread afterwards so we can review it here.
