didn't do that before - and it's not set for that., Still doing after cache cleared. Help

I have checked your form and the issue you described doesn't appear to be happening on my end.
However, I have noticed that your form has enabled its "Continue Forms Later" option from the form settings — How-to-Enable-Auto-Fill-Feature. If that feature is enabled, data that are stored in your browser's local storage when the browser is closed while filling up the form will pre-fill the form automatically when you access the form again.
You can prove this on your end by doing any of the following:
1. Submit the form and load it again — this is because the data in your browser's local storage only stays until the form is submitted.
2. Use Incognito or Private Mode of your browser — These modes don't store data in the browser storage. https://www.computerhope.com/issues/ch001378.htm
3. Clear your local browser's cache/data. http://www.refreshyourcache.com/en/home/
4. More than 24 hours have passed since the data were last stored.
