signing a pdf?

    how do you suggest i set up a form requiring a PDF be signed or at least signature agreeing to a PDF attached?

    i'm trying to avoid using Docusign. I have a patient file in a PDF and just need a secure way to acknowledge receipt of the attached PDF. i know i'd need to create a new form for each time i need to do this for a new patient, but how do i set this up?

    You could add any of the following signatures:

    And set it as required:

    That way your users will have to sign before submitting.

    Please note that firstly you will need to have a signature field in your form.

    If you already have a PDF fillable form, you can import that into JotForm and add the signature field in the PDF form - https://www.jotform.com/fillable-pdf-form-creator/. Please see the screenshots below.

    This way you can sign a PDF form. Hope this clarifies your query.
