Login failed- suspended

    Aloha!

    My account is showing it’s suspended? I haven’t done anything could someone please help me.

    I have checked your account and it was suspended due to the fields asking for credit card information on your form https://www.jotform.us/form/82598636984176, here is a screenshot of the guilty fields:
