hi I have a questionnaire that I want to allow my clients to download.  I want them to use a scale (1-4) to rate the statements I've given, then have their answers tallied for them to give them a final score.  The use the scoring information to see where they fit?

    Is this possible iwth Jotform?

    Hello Amanda,

I believe this is possible with Jotform. How are you going to get the final score, still through the form or are you going to do this manually right after receiving each of the entries?

Here's a demo form you can check using our Scale Rating field under Survey Tools: https://form.jotform.com/91565938919981.

The Total is our calculation widget (https://www.jotform.com/help/259-How-to-Perform-Form-Calculation-Using-a-Widget).

I hope that gives you the idea of how to create the survey form.

Please let us know if you need further assistance.

    Hi,

I want them to be able to download an actual file that they can keep and it will give them the calculation right on the download.  They will never send anything back to me, its just for their info only.    like a fillable pdf?

How much is it to get this form?

    Hi Amanda,

One option you can use is setting up the Autoresponder email to send the form data they filled out to the user. In the Advanced section of the Autoresponder is a PDF Attachment you can turn to "Yes", then the form data will be sent to all your users after submission of the form.

    Guide: Setting-up-an-Autoresponder-Email

Another option is to show the final score on the Thank You page after submission by adding the total score field to the Thank You page. Then the final score will show up on the Thank You page.

I also added a guide to set up your form will total for your calculation using the Form Calculation Widget.
