Prepopulate URL is not populating fields in the form

hello,
i have been trying since long but unable to populate my form fields using prepopulate URL.
i have used the Prepopulate App also but to fail
it should work but sadly it isn't
can somebody please tell me the fault ?

use password as : alpha
for demo purpose
i am using this above mentioned url in thank you page so that the next form which is same as this form gets autofilled with the same password.

It looks like you have changed the username for your account, so shared links do not work. Can you please provide us updated links so we checked them?

I dont understand but i have no access to my thread anymore.
Thanx for your time but it has started to work on its own.. maybe my browser issues or something.

i have not done anything, just changed some fields and it started working on its own.
