Form sections showing up twice and widget issues

The widget Short Scrollable terms does not want to accept when we try and check the box to agree to the terms.

In order to check the box for terms of the agreement, you need to read all the information inside the box. 

As you can see in the screen capture below, I read the information inside the box before clicking the checkbox.
