Can i build a quote form that references my own data sheet?

Hi there!

Looking for a form builder that will allow me to create a life insurance quote template using data that i have in an excel spreadsheet. The user answers questions and at the end gets their "rate". Is this possible with Jot Form?

You can integrate your Excel file with JotForm by using our Spreadsheet to Form Widget. The widget is filling form fields with the values in the Excel file according to a key field.
