Cannot find my forms when I logged in to my account

I logged into Jotforms to find both of my forms are now gone. I do not know why this happened. I am terrified that we have lost all of our submitted forms. Please, please, please help!

1. Your account username.

2. The email address associated with your account.

3. Form URL/link of any of your forms.

4. Email address used as email recipient of the email notification in your form.

5. The link to your website where your form is embedded (if it is embedded).
