To clarify, do you want to use your own PDF document or PDF form?

You may import your PDF form by following this guide:https://www.jotform.com/help/548-How-Can-I-Import-My-Own-PDF-to-JotForm

Please give it a try and let us know how it goes.

I would like to upload a PDF document that I can customize using the Jotform PDF form.

    Does that make sense?
