How can I create a sign-up sheet that looks like this?

    Hello,

     I would like to create a sign up sheet that looks like the attached form that will 1560645040Membership NE CC Volunteer She Screenshot 10allow people to sign up in the selected slots until they are filled up.

    Feel free to test and clone this Form.

    1. You can add 3 Appointment slots widgets for each of the dates shown. In each widget, you can add time slots.

    2. Next, add conditions to the Form to show the Appointment slots widget based on the selection of the date which would be configured in a dropdown:

    Do you want to allow each people to submit the Form once? By allowing this way the user would be able to also select only one of the slots.

    You can enable unique submissions in the Form.

    Unique Submission: You can take advantage of our unique submission feature to block multiple submissions from one system of one IP address by setting the "Check Cookies Only (Loose check)" or "Check Cookies & IP address (Strict Check)" respectively.     Using this widget you can limit how many people choose an available slot because the widget reduces the number of selected slots each time the form is submitted.
