Can you set up a form so that information entered in one section, can be populated as text in another area?

    I would like to set up a form that asks the client (check list):

    - what procedure they are booking in to have i.e. skin needling or cosmetic tattoo; and

    - what area they are booking for i.e. eyes, lips, face, neck


    At the bottom of the form I want this information to be populated as "skin needling - face" or "cosmetic tattoo - eyes brows" as examples.

    You can insert or pass data to another field by using a condition. First, here's my demo form:

    I used the Insert Text into a field using condition to pass the answer of the "Procedure" and "Target" single choice field to the "Final" field.
