I want to send a short self-service questionnaire to each customer where they can select a testing station

    Hi,

    I am investigating if your service would suit our needs. Please let me know if this is doable:

    Use case: we test cars for consumers and have many testing stations in the country. I want to send a short self-service questionnaire to each customer where they can select a testing station, upload some documents and answer a few standard questions. 

    We want each customer to be validated (or get a personal link) so we can present and store information to the right customer. We also want each customer to get a sub-selection of our 100+ stations and only see the 5 stations that are nearest to him. We have this information in our database but can we tailor the form to show this custom data? Without rebuilding the form for each customer.

    Essentially, can we: input situational data to populate the options/questions in the form according to a specific receiver - and store the answers to the correct id in our own database?

    After reviewing the details of your requirement, I believe what you were looking for is the new JotForm Mobile Forms "Kiosk Mode" feature.

    Please check also the user guide : How to Use Kiosk Mode in JotForm Mobile Forms

    Assuming you will use mobile device (preferably ipad/tablet - bigger screen) in the stations,the kiosk mode turns your device into a survey station.

    In locked kiosk mode, it allows access to only specific form and restricts access to other content, so you can easily and securely receive multiple submissions. It is a great choice for conferences, fundraisers, social events, trade shows, meetings, other types of events, onsite and offsite data collection.

    Hello,

    This was not really what I was asking for.

    1. I will send a form to the customer by email. And I will have hundreds of form links being processed at the same time on different locations and devices.
    2. The customer will get custom data in the form, that is appropriate to the specific customer.

    So, for example I would like the customer to receive a form that in the form says “Hello, Adam”. That is not my specific use case but the requested functionality.

    Is this possible?

    Thank you for the clarification and providing more details on the requirements.

    Base on the details you provided, I believe field prepopulator app will work.

    User guide: Prepopulating Fields to Your JotForm via URL Parameters

    You can open this demo form link in your browser. And you can see the form was already prepopulated with custom data specific only to the person receiving the form link.
