file uploaded returns a 404 error

Hello, 

I have a file upload for the Jot below.  However, we are getting an 404 error message when we are trying to view the file upload.

2019 US OPEN/ FATHER'S DAY BALL PROMO CREDIT

Please advise on how we can get to view the file.

I have checked the form submission but I can't seem to find the upload link or the submission data with the link you have shared.

Checked the other submissions and it seems the upload field is working.

You may let your form user edit his form submission and upload the files again.
