Our form stopped working as intended an hour ago - has been working properly for around 5 months

Our form https://form.jotformeu.com/80253139052348 suddenly stopped working as intended. Making choices and navigating through the different pages does not work as it previously did.

The form has not been modified since january - have you made any changes on the server side?
