I have a form that I am wanting to accept or deny

I have a form that I send out to customers to complete, when they send it in I want to accept or deny. Which I have sorted, but then on the back of this send them a reply with my decision either an accept email or deny email. Please can you tell me how I do this?

In case when email should be triggered with a change made in 'edit mode', please make sure that your emails have an option 'Send On Edit' enabled in email settings.
