How do I set up a form for patient examination so that it can be accessed by more than 1 provider?

We are conducting a health fair for underrepresented elementary school children (350 total).  There will be multiple providers needing access to the online (mobile) record.  How can I set that up?  For example, if I create the form and then number it so that there are 350, can each provider access each form and then fill out just their part of it?

I would suggest creating a form with general fields on it, do not add the 350 fields, if that's what you meant to create. Create the form, and let each provider submit one at a time. The form accepts multiple submission from the same user unless restricted (in the form settings).

Do you need each provider to access children data before they can submit the form? If yes, I'd suggest doing that outside the form, like provide them a spreadsheet with the data in it. If they decide to support the child, they will submit the form and provide the child data.
