Hi
I have many clients complain that when they access my form in their mobiles and complete filling the form and hit send button, it did not send and start over again with an empty form.
please help.

Your form is not set to redirect to any page, so not sure if there was a glitch or could be something your client reloaded the page accidentally. I tried the form on mobile and was able to submit it and get the thank you message page. I am not redirected.
Kindly check the form to your end, and let us know if you can replicate the problem. I'd also like to know if you have embedded the form on your website. If so, please share us the link.

I see. So, that is random. If you happened to get a report about this problem again, please ask how they fill the form and what are the things they did when filling the form and prior to getting the error on submit. Like if they've changed the selection, move back between fields, and if they get a sort of an error message in the form. This would help us troubleshoot the problem and find its cause.
