I am getting an error saying my IP address has been banned.

Hi!  I am getting an error saying my IP address has been banned for a possible phishing attempt.  I'm not sure why this is.  I am a teacher and am creating some forms to use for our school district and comparing them to what we currently use (Google Forms+Form Publisher).  I've tried to submit this numerous times but it keeps freezing up when I hit submit, so if this goes through multiple times, I'm sorry.  Is there a way we can get this issue fixed?  My username is lexi.commander
