Heading picture not showing in preview

I have finally managed to get my header picture to look perfect while editing the form, but it disappears when I try and preview the form. Please can you tell me what I have done wrong and how I can correct this

It appears the issue you are having is caused by the filename of the image you are trying to upload as the header image of your form. Please remove the apostrophe on the possessive name "Jacqueline's" on the filename and that should fix the issue you are having.
