IP blocked

    Hello and good morning!

    We’re a car dealership and mechanic shop that’s trying to make a credit application through JotForm. 

    I’m assuming the IP was blocked due to us asking for personal information, such as a SSN. We’ve added the terms and conditions and signature widget, to the form, so that the customer can authorize us having that information. 

    We’re currently trying to put this form on our website, but we cannot do this without our IP being unblocked first.

    We have checked your form and it was asking for SSN# numbers without the widgets: http://www.jotform.us/form/91687336879175 We needed to delete the collection in order to be able to reactivate your account.

    Your forms are available, please add the widgets for compliance at your earliest convenience.

    Our ip addresss is blocked and one of our staff members is unable to access the jotform webpage. Our ip address is 38.100.141.226. Can you please whiteliste it.
