Hi there, I work for the Tony Hawk Foundation - a NPO with a mission to foster lasting improvements in society with an emphasis on supporting youth in low-income communities through skateboarding programs and the creation of skateparks. We are launching a new campaign to seek out stories from our community on the impact of skateboarding and need a form that has upload capability for kids to include photos and videos and I have a few questions... 1) are your forms mobile friendly? 2) is there a max MB upload amount? and 3) Do you offer discounted NPO pricing?  I look forward to hearing back from you! Did you fill up the form completely and upload the needed document and then click the Submit button?

Oh, and one more question, I just created a JotForm primarily for the upload feature / compatible with mobile. Just tried to do a test upload and it won’t allow me to upload MOVs or MP4s. Is there a workaround for .MOV files as that’s the main reason we need to create a submission form. Thank you.

Yes, I filled everything out, attached the 990 form, pressed submit, and received an acknowledgement/success page. Please let me know what the issue could be.
Also, disregard the email re: .MOV and .MP4 files - I added those extensions to the permitted file types in the advanced settings.
