Forms are not syncing to Trello

I am getting an error screen when clicking 'submit' on forms when they are linked to Trello.

I checked all your forms and I did not see any trello integration. Could you please let us know to which form is this issue related to? Also, please reintegrate the form to trello and let us know if we can send a test submission to the form to test it from our end.

I have resetablished the form, Landscape Daily Report.

I receive this error now:

The error actually happens due to the field you have mapped with the name field from Trello, the field "Name and Job Reference" it's not supported on the Trello integration as a "Name" field, it's supposed to allow only a "Name" or "Short Text Entry" elements only, so you will need to replace the such field with any of the mentioned before, you will find them on the Add Form Element button, below the "Basic" section.
