I have clicked on the PDF attachment on the email settings and provided a strong password. no emails being sent when that is set. if I remove the PDF attachment it sends and email with no problem.
I found a few forum links that talked about this and they all ended with the problem should be fixed now. but I am still having issues.

I have tested a cloned version of your form using our HIPAA account and I can't seem to replicate the issue at my end.
Kindly try to create a new notification email and test the form again.
Please give it a try and let us know if the issue still remains.
