Sending a form letter to a specified email

Dear JotForm team,

I am trying to create form that allows mebers of the public to email a form letter with fixed text that cannot be changed to a specified email address.  Is this possible?

The dream scenario would be if you could add an extra field to allow submitters to add some comments of their own.
