As I check the form https://form.jotform.me/91414594450457, I see that there are no fields in the form. Could you check once again and let us know if you are referring to the right form? However, the form seems to be in the card layout. If you want to change the layout to classic layout, you may change it from the form settings.

Please note that there are no fields in the form "https://form.jotform.me/91414594450457". As requested, the form layout has been changed to the Classic form layout.

You can now configure the form to look similar to the other form. The easier way would be to clone the form "https://form.jotform.me/90491714911457" so as to obtain the similar format.
