Check out element not working

When I publish this form, the check out page to add a package is not working. It will not allow you to add any packages. I have tried everything from refreshing, trying a different computer, cloning the form, changing the settings (to not allow drop downs for packages), etc. every time I publish this, the check out page still seems to not work. 

I see that you have disabled one of the products. Please enable or delete the product to resolve the issue.
