Templates: Form to itemize forecast opportunities

I want each of our sales agents to provide a list, by account, of sales opportunities with the drop-down fields for each opportunity, with the drop-down options being different:

Name of account (free text)

business area (drop-down)

Platform(s) (free text)

Timing (drop-down to select month-year)

Probability (drop-down to select High, Medium, low)

Number of cases (free text)

Anticipated annual revenue (free text - numerical)

I can't find a template to leverage.

I think a Configurable list can come in handy with this particular requirement. This widget allows you to define the various field types per column on the list in a matrix-type of layout. Here's a quick demo for your reference:
