When I change the selection in field, it disappears.

    Also, I went ahead and changed the clear hidden field value to what you suggested and the fields occasionally continue to do the same thing. If I go through the form and select "English" and then a "therapy type" and then a "diagnosis", the field will remain. However, if I change the selection for the diagnosis to a different diagnosis, the entire field disappears again.

    Hello again,

    I think the problem is caused by the following condition:

    It is currently built to show itself based on self value. Please exclude field #20 from DO part of a condition and check your form again.

    Thank you. I did that and it's still doing the same thing. I'm not sure what else to do to resolve the issues I'm having with the form. Do you have any other solutions?

    I cloned your form and it seems you still have the condition to show the "Does your child have any of the following conditions?" if the same field is filled. Please edit the condition you have added for this field and remove it from the action field list, that will solve your problem. Please check the screenshot below:
