How to create a matrix field and show the responses in exported excel file?

How do I recreate this type of matrix in JotForm.  And secondarily, will each yes/no response show up as its own separate field when I export to Excel?

You can add an Input table field element into the form as shown below:

After that when you integrate the form with Google spreadsheet, you can get the responses in Yes and No columns - How-to-Integrate-Forms-with-Google-Sheets.
