What is the best way to use this form in a presentation setting?

Hello!

We want to use this form to gather information from many people during a presentation. Our goal is that we can have a link or something on our PPT slide and attendees can quickly get on their phones and fill out the form. 

What is the best way to go about doing this? I tried creating a custom URL, but I do not like that my name is unchangeable in the url and it is still too long.

I am assuming that you're referring to the custom Form URL in the Form Builder. You can request to change your account username to shorten the link. You can also use other sites (tinyurl or bitly) to shorten the form link.

You can provide the form link or shortened link to everyone and follow your presentation while filling out the form on their device.
