The issue seems to have been related to the form's cache, the form worked fine after clearing it.
Please kindly check and let us know if you continue experiencing the issue.

I am having this issue again. I've cleared the cache, waiting about an hour, and it's still happening. When will it be resolved?

I have checked your account and you're in the good status and far from your monthly form limits. However, the form you have shared seems to be deleted.
You other two forms however are working correctly.
You may try to check your trash folder for the form you have deleted if you wish to restore it.
