Account suspended

    Dear Jotform Support,

    This is Dharmesh from Hudson Dental Center. I have created new hire form however I believe I have SSN in there and hence it is suspended. Can you please activate my account and I will remove SSN and other sensitive information from the form. I am hiring few people and I would like to use this form.

    The form: was found in your as suspicious of phishing activity because you're collecting sensitive banking information and social security numbers:

    If it's important for you to process payments through bank transactions, consider the following recommendation. Jotform has implemented a way to help you process payments directly from your customers' bank account without infringing any law or putting our company in jeopardy.

    This payment processor is called eCheck.Net and you can learn more about this by following this guide on How to Integrate eCheck Net with your Form.

    Or, if your purpose is not to collect these data from the general public, but from your own customers, then you need to comply by adding this two mandatory widgets (this is also valid for the collection of Social Security Numbers):

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    We have unsuspended your account in order for you to be able to add the two above mentioned widgets for compliance.

    Please, make the appropriate modifications at your earliest convenience. If the forms are not found with these widgets in place the account could be subject for suspension again.

    Dear JotForms,
    I have made requested changes however Terms & Conditions widget was not
    showing the terms and conditions text I added. Instead, I have added
    regular text.
    Please confirm I can go ahead and use this form and my account wont be

    We appreciate your effort to collaborate. Nevertheless, the system will not recognize text for valid approval. The system is designed to screen the form for two widgets. If it finds just one of them, it will render the collection invalid and suspend the account.

    Try to copy/ paste to the scrollable widget. That one allows to retain external text and not a link forwarding to terms and conditions.
