I have been using Jot Form since July 2012...
Since last 3 - 4 months, my account has been upgraded to be premium monthly and I must pay $9 / Month
(before that, I got free)
What is the different in standard account and the premium one?
How come my account automatically upgrade to premium whereas I never asked to do that?
Are there any rules (or term and condition) that compulsory for any account in Jotform to be automatically updated to be premium after few months registered?
Can my account just make it standard, rather than premium?

Free and Premium account differ in features. You can compare both services and features here.
No, Jotform don't do automatic upgrade. You must have upgraded your account at some point and have forgotten about it.
