Form fields do not show even when there are no conditions set

    I have been working with this form for hours now. I tried to put several conditions on the form to show or hide certain fields, but nothing seemed to work. I finally removed all of the conditions and I still can not see several of my fields. Why not?

    Hi there, 

    I have cloned your form, numbered the fields on it and removed all requirements.

    Please explain the conditions that you would like to apply to the form.

    Example

    if 5 = whatever then SHOW 15

    See, even in this form there are several fields that are not showing up.
    There are a couple of fields at the bottom--including the submit
    buttons--that are not showing here. Also, field #19 shows Attendee #2.
    There are actually 8 more of these that are not showing up. I can see
    them when I edit them, but not when I preview them or see them on a website.
    This is puzzling--even on your cloned form there are about 11 or 12 more
    fields that you don't have on your form that are on the master form!

    Hi Dale,

    Please kindly check your form again: http://form.jotform.us/form/30595659739169, see if you still can't see the form fields. I have cleared your forms cache, if in case you wonder. This should fix delays to the changes made on your form.

    No, that did not help. There are still about 11 or 12 fields that are
    not showing--including the Submit button.
    I started over with a new form. I'll see how that works. So far so good.
    (I am assuming that somehow the original got corrupted.)
