Condition to show / hide form field is not working in form.

Hello I am trying to edit my main form

The attached keeps appearing in all my selection on the form footer, i need that to change as per the applicant choice, i did required edits in conditions but it keeps appearing. please advice 

I am not sure if I have understood your question correctly. Do you mean to say that you want to show the "Member Name" field only based on the selection of form field?

I did check your form and found that you have added 4 conditions to show multiple questions. Please let us know your condition requirement on when you want this question "Member Name" field to be displayed and in when you do not want this field to be displayed, we will try to help you with the condition.
